0-19 y/o (w/parent memb.): $27.50/mo
20-55 y/o: $165.25/mo
56+ y/o: $215/mo

Annual Payment Discounts

5% off when paid annually by check, debit card, or ACH bank transfer
2% off when paid annually by credit card

Membership pricing for nursing home or homebound patients is determined on an individual basis. Please contact our office to schedule a family meeting with one of our physicians to discuss the patient’s needs and pricing.
