Membership Fees include primary care services, concierge benefits, and nutrition and life coaching:
***Pricing effective October 1st 2019***
| 0-19 years old: | $76.50 / month |
| 0-19 years old: | $26 / month with at least one parent membership |
| 20-55 years old: | $153 / month |
| 56 years and older: | $199 / month |
***Price increase as noted below will be effective as of February 1st, 2024***
| 0-19 years old: | $82.50 / month |
| 0-19 years old: | $27.50 / month with at least one parent membership |
| 20-55 years old: | $165.25 / month |
| 56 years and older: | $215 / month |
***5% discount off monthly rate for membership fees paid annually by check, debit card or ACH transfer from bank account***
***2% discount off monthly rate for membership fees paid annually by credit card***
Nursing home / home bound patients pricing will be determined on an individual basis. Please contact the office to set up a family meeting with one of our physicians to discuss patient’s needs and pricing.
